Help on accessing PDF forms
The Internet browser that you use may affect your ability to access PDF forms. Try using Internet Explorer 11+. If you are having difficulty, follow these instructions.
To access these PDF forms:
- Use your computer as the forms won't open on mobile devices
- Install Adobe Reader 10 or higher
- Download and save the PDF file to your computer
- Make sure you use Adobe Reader to open the form as sometimes if you try to open the form directly, your Internet browser will try to open it
